At Journey Occupational Therapy, we offer a range of community based services - we travel to you to provide Occupational Therapy in the comfort of your home. Functional Capacity Assessments (FCA)

 

Journey Occupational Therapy can complete functional capacity assessments with you, to help get a clear understanding of where you are currently, where you want to be in the future, and what supports are needed to get you there.

FCA’s are all about getting a comprehensive understanding of you, your current goals, the support you need in day to day life and what intervention and support is needed to help you get to wherever you want to go on your journey. FCA’s can be completed in-person, or via telehealth, and will often involve multiple assessment sessions, and completion of standardised assessments to help create a report that outlines your current support needs and goals.

Specialist Disability Accomodation (SDA) Assessment

 

Navigating the NDIS can be tricky at the best of times, at Journey Occupational Therapy we can help you to explore different options for accommodation under the scheme and provide assessment and recommendations for each of the SDA design categories, depending on your need:

  • improved liveability

  • robust construction

  • fully accessible

  • high physical support.

Supported Independent Living (SIL) Assessment

 

SIL is a way of making sure you are able to live with the level of support that you need to live however you want to live. SIL is often for people that need a significant level of support, 7 days a week. At Journey Occupational Therapy we can help work with you and your support team to explore independent living options, the level of supports you need in your day to day live to help you:

  • pursue your goals

  • improve or sustain your functional capacity, helping you to do more things with less support

  • reduce or sustain your need for person-to-person supports

  • create better connections with your family, community, health services, education and employment.

We can also help you investigate other support options, and provide assessment, reporting and recommendations to help you get the support you need.

Assistive Technology

 

Journey Occupational Therapy can help you navigate the confusing and overwhelming world of assistive technology, provide expert advice, assist with trials, and reporting to connect you with the equipment you need.

There are so many technologies and pieces of equipment out there that can help individuals with a huge range of activities. Journey Occupational Therapy can help provide education and coaching to help you make the right choice regarding equipment that fits your need. We can complete comprehensive assessment and reporting, and follow up services to ensure equipment is working as intended and you and your support team are comfortable with how to use the equipment or technology you need.

Home Modifications

 

Small changes to your home environment can make significant changes to how easy it is to complete the activities that you need to at home. Journey Occupational Therapy can provide assessment and reporting, as well as assisting with quotes, liaising with builders and exploring suitable home modification options to make sure that your home environment is limiting your ability to live.

If the work is outside of the scope of Journey Occupational Therapy, we can help connect you with other specialised Occupational Therapists to ensure your home modification needs are met.

Cognitive Assessment

 

Illness, injury or disability can affect the way you think, process thoughts, and solve problems.

Journey Occupational Therapy can help you by providing cognitive assessment and screening, intervention where appropriate, and referral to specialised cognitive services when needed.

Occupational Therapy Intervention

 

Occupational Therapy capacity building intervention, to help you restore function lost due to illness or disability, compensate for changes in function and provide a coaching and education. Journey Occupational Therapy will work with you to help reach your goals, whatever they are.

Education, Coaching and Training

 

Alongside other services, Journey Occupational Therapy are passionate about providing comprehensive education, coaching and training to you or your support team, to make sure you are able to make informed decisions about your own health and well-being.
